WHAT IS COMMUNITY FARMACY?

IMPROVING HEALTHY CHOICES * DECREASING PROCESSED FOOD * IMPROVING DIABETES

Community Farmacy is a program developed to help patients struggling with diseases, like Type 2 diabetes, to take control of their health. Participants will learn to make healthy lifestyle and food choices with medical, dietician, and food support. The program is a completely voluntary 12 week program of Community Harvest Food Bank, in partnership with Matthew 25 Health and Care. The purpose of this program is to help prevent the progression of diabetes and hopefully have participants feel better. This is done by using fresh foods as medicine to improve healthy choices.

This partnership is to provide whole food as medicine along with nutritional education with the intent to improve health outcomes including Type 1 and
Type 2 Diabetes and obesity for the undeserved population.

The aim of the program is that participants will learn how food can help reverse the course of disease, decrease unhealthy processed food consumption, and feel good about making health choices going forward. The benefits of the program include possible improvement of diabetes, weight reduction, increased understanding of diabetes, free whole foods (produce, fruit, and meats), and free prizes for completing various levels of the program.

MISSION STATEMENT

To provide fresh produce and nutritional education at no charge to participants. The benefit is to provide whole food as medicine to improve health outcomes including pre-diabetes, Type 2 diabetes and obesity for the underserved population.

COMMUNITY FARMACY GOALS

• To increase fresh vegetable and fruit consumption for those having diet related illness’
• To reduce barriers of access and education for vegetable and fruit consumption
• To build awareness on the utility of Community Harvest and Matthew 25 Clinic as tools to reduce reliance on medications and to help prevent progression of diabetes mellitus
• To improve the quality of patient’s lives (e.g., feeling more active) and reduce the burden of disease
